Many, many years ago I was working on a Naval Establishment on the South Coast. I had to run cabling to a new piece of equipment, which meant running the new cables through existing 12" metal trunking fixed to the wall. This entailed cutting a piece of the trunking off to fit a new piece at 90 degrees to the original run. Being too lazy to take the trunking completely off the wall, I pulled all the cables out of the metal trunking and drilled a hole so that I could use a Jigsaw to make the cut. managed to cut one side of the required hole - no problem! - Just finishing off the second cut and suddenly, "*BANG*"!!!! followed by a cascade of sparks, balls of molten metal showering down on me and me beating a hasty retreat from the room.
Needless to say, I'd missed pulling out of the trunking three 415V 60A(!) three phase single core cables that had been taped together. The jigsaw blade was welded to the trunking, the jigsaw itself never ran again, I needed new underpants, and worst of all... The whole establishment lost electrical power as I'd blown the Main Incoming Fuses which took a day to deliver and replace! The Navy were not too pleased as they were playing wargames with a French frigate out in the English Channel at the time.....Game Over!
For some reason I wasn't allowed on site for the next 6 Months...I wonder why?

In a factory (1963) I was de-burring small lengths of wire to make plating hangers, several lengths in my hand at a time, and, my thumb on my left hand got pulled onto the wheel, ground all the flat of the nail off, arm elevated in sling for 6 weeks, to this day I get little black bits grow out under the nail.

I used to work in a machine shop with all sorts of equipment including a very large bandsaw. One of the guys had been working 12 hours all week and it was last thing on a Friday. He was blowing swarf onto the conveyor to clean the saw down. The conveyor only ran whilst the saw blade was running. The gloves he was wearing touched one of the teeth of the blade and whipped his hand to one side, where the blade passes the block the blade the got a proper grip and took his hand straight off! 1 Ambulance and 2 Police outriders took him to Wyhtenshaw Hospital and 12 hours later it was back on again. That was his last day of work. I did bump into him a couple of years later in B and Q he held out his hand and I held out mine and he said "Dont shake it too hard!" I was amazed at the job they did getting it back on. Only his little finger didn't work quit rightly. But the rest was fine.
